The Diabetes card lets people know how to support you when you need to be. Personalise it by adding your details onto the back.
The card comes with the choice of a FREE standard or small sized Sunflower lanyard with as well as a plastic wallet to protect the card.

Make the Sunflower Diabetes card your own by adding your details onto the back. The card is available as a waterproof paper card or for an additional cost, you can choose a durable plastic card when you add it to your shopping basket.
On the back of the cards, you can:
1. Add your photo - upload a high-resolution, clear photograph of yourself
2. Add your name - add the name that you want people to use when addressing you
3. Add icons - use one of our icons described here. You can add up to five.
4. Add a contact - add a name and number to be contacted with your consent or in an emergency
